Outcomes
Every single person that attends Assist will have their own co-produced Outcome Plan.
The Assist Outcome Plans will describe all the progress and plans for the future that each member is working on and these are updated 3 times a year as each new timetable starts. Assist Keyworkers will help each member to produce the Outcome Plan itself and families and carers are also consulted regularly as long as the individual agrees to having that input.
